Posttraumatic stress and delay discounting: a meta-analytic review

Delay discounting—the extent to which individuals show a preference for smaller immediate rewards over larger delayed rewards—has been proposed as a transdiagnostic neurocognitive process across mental health conditions, but its examination in relation to posttraumatic stress disorder (PTSD) is comparatively recent. To assess the aggregated evidence for elevated delay discounting in relation to posttraumatic stress, we conducted a meta-analysis on existing empirical literature. Bibliographic searches identified 209 candidate articles, of which 13 articles with 14 independent effect sizes were eligible for meta-analysis, reflecting a combined sample size of N = 6897. Individual study designs included case-control (e.g. examination of differences in delay discounting between individuals with and without PTSD) and continuous association studies (e.g. relationship between posttraumatic stress symptom severity and delay discounting). In a combined analysis of all studies, the overall relationship was a small but statistically significant positive association between posttraumatic stress and delay discounting (r = .135, p < .0001). The same relationship was statistically significant for continuous association studies (r = .092, p = .027) and case-control designs (r = .179, p < .001). Evidence of publication bias was minimal. The included studies were limited in that many did not concurrently incorporate other psychiatric conditions in the analyses, leaving the specificity of the relationship to posttraumatic stress less clear. Nonetheless, these findings are broadly consistent with previous meta-analyses of delayed reward discounting in relation to other mental health conditions and provide further evidence for the transdiagnostic utility of this construct.
Relapse prevention through health technology program reduces hospitalization in schizophrenia

Background
Psychiatric hospitalization is a major driver of cost in the treatment of schizophrenia. Here, we asked whether a technology-enhanced approach to relapse prevention could reduce days spent in a hospital after discharge.
MethodsThe Improving Care and Reducing Cost (ICRC) study was a quasi-experimental clinical trial in outpatients with schizophrenia conducted between 26 February 2013 and 17 April 2015 at 10 different sites in the USA in an outpatient setting. Patients were between 18 and 60 years old with a diagnosis of schizophrenia, schizoaffective disorder, or psychotic disorder not otherwise specified. Patients received usual care or a technology-enhanced relapse prevention program during a 6-month period after discharge. The health technology program included in-person, individualized relapse prevention planning with treatments delivered via smartphones and computers, as well as a web-based prescriber decision support program. The main outcome measure was days spent in a psychiatric hospital during 6 months after discharge.
ResultsThe study included 462 patients, of which 438 had complete baseline data and were thus used for propensity matching and analysis. Control participants (N = 89; 37 females) were enrolled first and received usual care for relapse prevention followed by 349 participants (128 females) who received technology-enhanced relapse prevention. During 6-month follow-up, 43% of control and 24% of intervention participants were hospitalized (χ2 = 11.76, p<0.001). Days of hospitalization were reduced by 5 days (mean days: b = −4.58, 95% CI −9.03 to −0.13, p = 0.044) in the intervention condition compared to control.
ConclusionsThese results suggest that technology-enhanced relapse prevention is an effective and feasible way to reduce rehospitalization days among patients with schizophrenia.

Despite the strong link between childhood maltreatment and psychopathology, the underlying neurodevelopmental mechanisms are poorly understood and difficult to disentangle from heritable and prenatal factors. This study used a translational macaque model of infant maltreatment in which the adverse experience occurs in the first months of life, during intense maturation of amygdala circuits important for stress and emotional regulation. Thus, we examined the developmental impact of maltreatment on amygdala functional connectivity (FC) longitudinally, from infancy through the juvenile period. Using resting state functional magnetic resonance imaging (MRI) we performed amygdala–prefrontal cortex (PFC) region-of-interest and exploratory whole-brain amygdala FC analyses. The latter showed (a) developmental increases in amygdala FC with many regions, likely supporting increased processing of socioemotional-relevant stimuli with age; and (b) maltreatment effects on amygdala coupling with arousal and stress brain regions (locus coeruleus, laterodorsal tegmental area) that emerged with age. Maltreated juveniles showed weaker FC than controls, which was negatively associated with infant hair cortisol concentrations. Findings from the region-of-interest analysis also showed weaker amygdala FC with PFC regions in maltreated animals than controls since infancy, whereas bilateral amygdala FC was stronger in maltreated animals. These effects on amygdala FC development may underlie the poor behavioral outcomes associated with this adverse experience.

Summary
Nearly three times as many people detained in a jail have a serious mental illness (SMI) when compared to community samples. Once an individual with SMI gets involved in the criminal justice system, they are more likely than the general population to stay in the system, face repeated incarcerations, and return to prison more quickly when compared to their nonmentally ill counterparts.

The Cal-DSH Diversion Guidelines provide 10 general guidelines that jurisdictions should consider when developing diversion programs for individuals with a serious mental illness (SMI) who become involved in the criminal justice system. Screening for SMI in a jail setting is reviewed. In addition, important treatment interventions for SMI and substance use disorders are highlighted with the need to address criminogenic risk factors highlighted.

Here we provide comprehensive guidelines for the assessment and treatment of violence and aggression of various etiologies, including psychotic aggression and impulsive aggression due to schizophrenia, mood disorders, ADHD, or trauma, and predatory aggression due to psychopathy and other personality disorders. These guidelines have been developed from a collection of prescribing recommendations, clinical trial results, and years of clinical experience in treating patients who are persistently violent or aggressive in the California Department of State Hospital System. Many of the recommendations provided in these guidelines employ off-label prescribing practices; thus, sound clinical judgment based on individual patient needs and according to institution formularies must be considered when applying these guidelines in clinical practice.

Summary
HANS-JOACHIM SCHOEPS AND MY FATHER, Karl Otto David Meyerowitz, were each born in East Prussia in 1909, my father in Königsberg, home of the Jewish Reform Movement inspired by Julius's ancestor Moses Mendelsohn. The intellectual and religious currents unleashed in the Jewish Enlightenment or Haskalah in the eighteenth century were still being played out in the lifetimes of both our fathers, in my family's case with a little different script but with the same defining characteristics. It may be instructive, then, to briefly sketch the story of another prominent German Jewish family from this milieu. It may provide a richer context for considering the perplexing case of Schoeps.
My father was born into an aristocratic Prussian Jewish family like the original Mendelsohns: his father was a wealthy textile factory owner with a house on the sea and a schooner with a captain and mate as well. Opa Benno Meyerowitz married Oma Kaethe Supplee, a Protestant Huguenot whose ancestors were from St. Petersburg and who, when Hitler came to power, converted to Judaism as a sign of resistance and family unity. She was a concert pianist and accomplished soprano, Benno served as a volunteer impresario of the Königsberg State Opera, and the family spent several nights a week either in the front row of the opera house or backstage watching the action unfold. My father knew Puccini and Verdi in German and studied voice while preparing to take over the family business after attending university. He had two older sisters, Eva and Leni, and their fateful lives also illuminate the social and intellectual world of Prussian Jewry in the 1920s and 1930s.
In 1922 my father became a Bar Mitzvah in the Great Liberal Synagogue of Königsberg where men and women sat separately and the men on such occasions wore cutaways and high hats. The famous cantor and composer, Louis Lewandowski, who died there in 1894, wore religious garb as well as a high yarmulke, and could have been mistaken for a Lutheran pastor. While the holidays were celebrated at home after a fashion, the Jewish calendar did not greatly influence the lives of the Meyerowitz family.

PRESERVATION AND ADAPTATION: THE ENCOUNTER WITH HELLENISM
Alexander and his successors
In 333 b.c.e. Alexander the Great defeated Darius the Persian and gained access to both Egypt and Mesopotamia. A year later he took Egypt as well as the Mediterranean coast, and Jerusalem, along with all the other cities of the region, submitted. With the conquest of the eastern Mediterranean the Hellenistic period begins. Despite reports in Josephus and later Rabbinic legend, Alexander did not visit Jerusalem. But his battles did take him to Persia, which he finally subdued along with Mesopotamia and surrounding territories as far as the Indus River (in present-day Pakistan). Alexander's untimely death in 323 left his unstable and scattered conquests in the hands of his fractious generals, but the cultural and political changes that took shape in his wake prevailed. The “Hellenistic period” would last until the growing influence over and the conquest of the area by the Romans between 200 and 31 b.c.e.
One of Alexander's generals, Ptolemy, used Egypt's natural borders and wealth to consolidate his rule there and, in 305 b.c.e., declared an independent kingdom. Another general, Seleucus, who ruled Mesopotamia from Babylon, fended off numerous attacks from both indigenous populations and a third general, Antigonus, who controlled Asia Minor (modern Turkey). After surviving exile from his own kingdom, Seleucus finally consolidated his control in 301 b.c.e. when he and a coalition of local military leaders defeated Antigonus at Ipsus in western Asia Minor.

THE WORLD OF ISRAEL'S ANCESTORS (THE BOOKS OF GENESIS THROUGH DEUTERONOMY) AND OUR WORLD
The first five books of the Bible describe the creation of the world (Genesis 1–2) and the earliest history of humanity including the destruction of the inhabited world by a flood, before they tell the story of the ancestors of Jacob, who came to be called “Israel,” which means “he struggles with God.” The story includes the call of Abraham to come to the land of Canaan (Genesis 12) and continues with the lives of Isaac and Jacob. Jacob is named Israel as the consequence of his physical struggle with God (Genesis 32), and he gives that name to the people called Israel. The twelve sons of Jacob/Israel go down to Egypt; after their descendants become a sizable people, they are forced into slavery by a pharaoh. Moses, one of the “Hebrews” (as the Egyptians call these foreigners with their strange, West Semitic language) brought up at the Egyptian court, leads the Hebrews out of slavery and through the wilderness of Sinai to the threshold of the land of Canaan. On the way, Moses gives the people, on behalf of God, civil, religious, and ceremonial laws that identify the Hebrews as the people of Yahweh, who first called Abraham, Isaac, and Jacob and who delivered the Hebrews from slavery.
